What Is Healthy Families?
Healthy Families is an early intervention home visitation program
designed to support families in providing a nurturing, stimulating and safe
environment for their children from birth to the age of five. Based on a review
of their situation, families with new babies will be offered home visiting
services by trained staff in a way that meets their specific needs.
How Does The Healthy Families Program Work?
Step 1 - Interview and Referral
Families can be referred to the Healthy Families program by their Public Health
Nurse, community agencies, family and friends, or by the families themselves.
During the initial interview with the family, a checklist will be completed to
identify those families who qualify for the program. This interview can be
completed in person or over the phone, either prenatally or up to three months
(or 12 months in special circumstances) after the birth of the baby.
Step 2 - Review
A Healthy Families Home Visitor will come to your home for a review of
your family’s situation. This first visit will identify those families who would
benefit most from home visiting, and how the program can build on the families’
strengths.
Step 3 - Home Visiting
Weekly home visits begin after the birth of the baby, or during the eighth month
of pregnancy. As the family progresses, the visits will occur less frequently.
Being a voluntary program, the family can choose to discontinue service at any
time.
